What you can build
The FieldWaiver form builder lets you create a waiver, release, registration or check-in form from your own wording and the fields you need. Add fields, drag them into order, and set each one as required or optional with an optional description. Section fields split a long form into pages with their own headings, and static text blocks hold your terms or safety briefing.
If you would rather not start from a blank page, five templates are built in: Activity Liability Waiver, Photo / Video Release, Event Vendor Form, Equipment Rental Agreement and General Consent Form. Treat them as starting points and replace the wording with your own.
Field types

How it works in practice
- Create a form, either blank or from a template.
- Add and order your fields, mark the required ones and paste in your waiver wording as static text.
- Add conditions so follow-up questions only appear when relevant.
- In form settings, turn on a consent checkbox with your own wording, set up notification and receipt emails, and choose how long to keep submissions.
- Fill in a test submission on a phone or tablet before using it with the public.
When it matters
Every operator's risks are different. A go-karting track needs height and experience questions, a water sports centre needs swimming ability, and an event needs vendor and insurance details. Building your own form means you are not forcing participants through someone else's generic waiver. FieldWaiver does not assess your wording, so have your terms reviewed by your own adviser.
Limits and plan availability
The number of forms depends on your plan: 1 on Free, 5 on Starter, and unlimited on Professional and Enterprise. Image and file upload fields default to a 10MB maximum per file, and uploads count towards your plan's storage allowance.
Common questions
Do participants need an account to fill in a form?
No. Share a public link and participants complete the form in their browser. Staff collecting on their own devices sign in.
Can a form have more than one page?
Yes. Add a section field wherever you want a new page to start.
Can several people sign one form?
Yes. Put the participant fields, including a signature, inside a repeatable group and each person gets their own entry.
